2. 2nd check for $25 to Ohio Judo.

The Background Application has a separate fee and should be mailed (with a check
for the fee) to the address on the form, NOT to Ohio Judo.

Application_for_Sanction_2008.pdf. Fill out the form. Please
include 1 check to USA Judo based on the # of expected competitors and include a
2nd check to Ohio Judo for $25.
Please mail your Application for Sanction and 2 checks (Check #1 to USA Judo and
Check #2 to Ohio Judo) to:
